What is the document for?

This document is needed for deregistration at the place of previous residence due to relocation. If the act of departure has been lost, the citizen must contact the Ministry of Internal Affairs at the new place of residence with a written application and detail all the circumstances.

The absence of a departure certificate is not an obstacle to filing an application for a new place of residence.

Russians are also deregistered:

  • at the hotel;
  • in the clinic;
  • at a tourist base;
  • in any place not intended for long-term residence.

If your stay was short, you do not need to declare deregistration. This only needs to be done if the citizen wants to leave earlier than planned.

When a person is discharged from the hospital, the departure certificate is issued by authorized hospital employees.

Receiving data on form No. 9

Official documentation showing the number of registered Russians is required:

  • when making real estate transactions;
  • to confirm registration;
  • when registering an inheritance;
  • when privatizing residential premises;
  • in other cases.

A certificate of those registered in the apartment is issued free of charge upon application. The registration time does not exceed 20 minutes. MFC has the right to issue Form No. 9.

A certificate of registration is issued to residents who have received registration in the apartment about which data is required. Having figured out where to get it, you should find out what documents are needed to provide information:

  • requesting that a certificate f9 be provided;
  • passport of the applicant.

House management passport officers often require confirmation of the absence of debt on utility bills. This condition is illegal. Form 9 registration certificate is issued to applicants without delay upon presentation of the required documentation. In case of refusal to issue documentation at the place of residence for utility debtors, the following procedure is provided for challenging the actions of the passport officer:

  • requirement of a written refusal, indicating why a certificate of registration is not issued;
  • complaint to the FMS manager;
  • going to court.

The legislation does not limit the expiration date of documentation. But most organizations require that the registration certificate Form 9 be issued no more than 2 weeks ago.

A certificate of registration at the place of residence to confirm registration displays:

  • who applied for the issuance of documentation;
  • area of ​​the premises, number of rooms;
  • the number of registered persons, indicating the degree of relationship with the applicant.

Archival certificate Form 9 contains information about all residents registered at the specified address. Where can I obtain such documentation?

It is issued at the passport office in the area where the residential premises are located.

What does a leaf look like?

The form of the document was regulated by Russian legislation. It must be filled in on both sides. The form contains a machine-readable data block that does not need to be filled out.

Citizens of the Republic of Belarus who wish to live and work in Russia need a Form 20 departure slip.

In Minsk and regional centers, this document is drawn up in 3 copies. In other settlements - in 2 copies.

After marking the departure, one copy is returned to the applicant along with an identification .

We fill out the act of acceptance of materials in form M-7

The form consists of 4 sheets, each of which must be filled out.

First sheet

First of all, indicate the following information:

  • act number;
  • name of company;
  • OKPO code;
  • date of drawing up the act;
  • place of compilation;
  • start and end times of materials acceptance (indicate hours and minutes);
  • information about accompanying documents;
  • name, address, telephone number of the sender, supplier, recipient, insurance company;
  • date of dispatch of materials from the sender's warehouse;
  • details of the supply agreement.

Next is a table where you need to indicate how many and what products were listed on the accompanying transport documents. All this data must be taken from the supplier’s invoice or other documents.

So, you need to enter into the table:

  1. A mark on the sealing of the cargo.
  2. Number of seats.
  3. Type of packaging.
  4. Product name or container numbers.
  5. Units.
  6. Weight of cargo.
  7. Special notes from the sender on the waybill.

Then there is a table with information about the date and time when the cargo arrived at the destination station, was issued, a wagon or other vehicle was opened, and the cargo was delivered to the warehouse of the recipient company.

Second sheet

Here it is necessary to describe the conditions for storing materials in a warehouse or other premises of the recipient organization, in what condition the containers and packaging were at the time of inspection of the products. Next, you need to indicate by what method the quantity of missing products was identified: weighing, counting, measuring, etc.

Third sheet

Here you need to enter data about defects, shortages or surpluses of products. First, fill out a table with information such as transaction type code, structural unit, type of activity, warehouse, supplier code, corresponding account, and accompanying document number.

The second table should contain the following information:

  • name and nomenclature number of material assets;
  • units of measurement (name and code);
  • actual data and documents on the type, quantity, price and amount of goods;
  • serial number of the record according to warehouse documents;
  • data on defects, damage, as well as shortages and surpluses (quantity, amount);
  • passport ID.

Fourth sheet

On this sheet the selection committee writes its conclusion and contains a list of attached documents. Next, the members of the commission put their signatures, in addition, indicate the details of the documents on the powers and the name of the organization that issued them.

At the very end, they put the date when the material assets were accepted and capitalized, indicate the details of the commercial act, and the warehouse manager puts his signature.

Attention! The act must be approved by the head of the organization receiving the cargo. He indicates his position and signs on the first sheet in the upper right corner.

How to fill out the form

It is advisable to fill out the form on a computer. This is much faster and more convenient for both the applicant and authorized employees. But you can print the form and fill it out by hand.

There are 15 questions on the departure sheet. On side “A” there are points from 1 to 11. Answering them is mandatory.

On side “B” there are points 12 to 17. Point 12 is mandatory. Not everyone needs to answer the remaining questions.

Procedure for marking departure:

  1. Contact a department of the Ministry of Internal Affairs.
  2. Fill out form 7.
  3. After filling out the departure address sheet in Form 7, the authorized employee puts the date and certifies the document with a personal signature. This means that he has no comments on the information and documents presented.
  4. After completing the procedure, the Russian citizen is returned with a stamp.

The mark is affixed by the person who is responsible for receiving and transmitting documents to the registration authorities.

Is it necessary to have a departure slip?

The Ministry of Internal Affairs deals with migration issues in Russia. The procedure for deregistration at the place of residence is established by Order No. 288 of the Federal Migration Service of Russia dated September 11, 2012. Thus, paragraph 31 of the Order establishes the right of citizens upon arrival to a new place of residence to provide Form No. 7, issued at the previous address of residence. It must be issued by the person responsible for receiving and transmitting documents to the regulatory authorities. But the presence of such a certificate is not necessary, and refusal of registration in its absence is illegal. Therefore, if you do not have this document, the migration service at your new place of residence will make two notes in your passport: about departure and about arrival.

What to do if you lose a leaf

If a Russian has lost his departure certificate, Order of the Ministry of Internal Affairs of the Russian Federation of October 23, 1995 No. 393 (Extracts) applies to him.

The procedure for restoring a sheet:

  1. Contact your department of the Ministry of Internal Affairs with a statement.
  2. Fill out a new form in two copies.
  3. Submit a new sheet, be sure to clarify that it needs to be restored if lost.

A new sheet is issued to the applicant within a week. It is certified by the seal of the Ministry of Internal Affairs.

Why do you need a departure slip and where to get it?

The departure address sheet (form 7) is included in the list of documents developed by the Main Directorate for Migration of the Ministry of Internal Affairs of Russia (formerly the Federal Migration Service) for the registration procedure at a person’s place of residence/stay in the Russian Federation.

Registration is carried out by the Main Directorate for Migration of the Ministry of Internal Affairs of Russia. To obtain an extract, you must contact the local migration office or any of the government bodies that provide intermediary services between the citizen and the migration service:

  • passport office at the housing office;
  • MFC;
  • State Services portal.

Important ! Only during the self-discharge procedure does a person receive a departure address sheet.

When a minor is discharged, all documents are drawn up for him by his parents, adoptive parents or legal guardians, subject to permission from the guardianship authorities.

At the new place of residence, a person submits his passport for registration along with the departure address sheet.

Questions and answers

Will they accept my documents for my new place of residence without Form 7?

Yes, they will. Previously, without this document, you could have been denied registration. But now the Regulations have been changed in your favor: documents are accepted, but the request is sent to your previous place of residence. The employee will provide you with a written explanation.

What if I lost both my departure slip and my passport?

In this case, a duplicate will not be issued. You need to apply for a new passport.

They don't register me. What should I do?

Form 7 documentation

Russians often require documents in forms 7 and 9. Having figured out how to obtain a certificate of form 9, it is easy to figure out how to obtain form 7. The document characterizes residential premises and is required when making a number of transactions:

  • obtaining a mortgage loan;
  • registration of inheritance;
  • sale of housing.

Form 7 certificate is issued at passport offices.

The validity period of the certificate is not limited, but many organizations accept Form 7 from passport offices, issued no more than 7-30 days ago.

Certificate Form 7 contains the following information:

  • footage of the home;
  • apartment floor;
  • number of rooms;
  • redevelopment carried out;
  • defects found in the premises;
  • availability of communications;
  • carrying out the last major renovation of the house.

Certificate Form 7 is a shortened version of the apartment registration certificate issued by the BTI.

Removal from the register

In accordance with the current legislation, the place of residence of any citizen is considered to be a certain housing in which a person resides on a permanent or temporary basis, and in the event of a change of residence address, one must first deregister at the previous one, and only then register at the new one.

To carry out this procedure, you need to contact the government agency responsible for processing and issuing the relevant documents by submitting an application. After completing all the documentation, the citizen is issued a passport, which contains a note about the discharge, as well as a certificate of departure from the previous place of residence.

In this case, there are two options for carrying out this operation - submitting an appropriate application when moving or after arriving at a new address, where you can issue an extract along with your registration and, thus, be deregistered completely automatically.

In the first case, it is enough to contact the government agency located at your previous place of residence and submit an application along with your passport, then wait for the departure certificate to be issued and receive it in your hands.

The second option has a lot of advantages, since a person does not have to come to the passport office twice to register, and it is quite enough to simply visit this authority at the new place of residence and contact the appropriate authority, after which you will immediately receive both a departure certificate and documents confirming the registration of the new registration.

Statistical form 7-injury – who takes it and when

The form is submitted once a year. Such reporting is provided only for legal entities; individual entrepreneurs and self-employed persons do not submit it. Micro-enterprises may not submit this report. By type of activity, the following organizations also fall into the category of exceptions (that is, exempt from this injury reporting):

  • conducting financial activities;
  • Insurance companies;
  • government bodies and those providing military security;
  • organizations working in the field of social services;
  • educational institutions;
  • households are employers engaged in the production of goods;
  • extraterritorial companies.

Information on injuries at work (form 7-injuries) is submitted to the territorial divisions of Rosstat with reference to the location of the business entity. Reports for the past year must be submitted no later than January 25 of the following year. Additionally, once every 3 years, enterprises must fill out an appendix to form 7-injury, which indicates information about the causes of accidents that occurred (the form was approved by Rosstat Order No. 493 dated August 10, 2018).

Form 7-Injury can be submitted in the following ways:

  • in paper format by personal appearance of a representative of a legal entity;
  • a paper report can be submitted to the Rosstat office through a proxy (provided that the authorized individual has the original power of attorney and passport);
  • a report drawn up on paper can be sent to Rosstat by mail - the letter must be registered with acknowledgment of receipt, an inventory must be included in the envelope, certified by the signature of an employee of the post office and a postmark with the date of dispatch;
  • an electronic document can be submitted via TKS.

In what cases is it necessary

Registration of a departure address sheet in Form No. 7 is carried out if a person temporarily registered at a certain address intends to leave it before the deadline established in accordance with the certificate of temporary registration.

If a person is discharged from any medical institution, the authorized person of this organization, within three days from the date of discharge, must draw up a departure address sheet in form No. 7 in two copies, after which one of them is sent to the appropriate address and reference unit of the nearest territorial body, while the second one will be sent to the registration authority.

If the migration service or its individual territorial bodies refuse to grant citizens the status of forced migrants, then after three months from the date of registration of these citizens at their place of residence, they are deregistered, and the corresponding stamp is placed on the second copy of the departure slip .
