What is the document?

Certificate form 070/у is a completed medical form that contains information about recommended treatment measures and procedures in sanatoriums.
The medical certificate also contains information about which procedures are not recommended for the patient. Attention! A certificate for obtaining a voucher to a sanatorium is issued free of charge at the clinic at the patient’s place of residence or registration.
It is also possible to obtain a medical form from a private clinic, but then you will have to pay for the service. The procedure for filling out the card, the type and content of form 070/u are established and secured by Order of the Ministry of Health of Russia No. 834n dated December 15, 2014. According to this Order, a certificate of the established form can be issued only by medical organizations that have the appropriate license.

The certificate can be provided in two forms: paper and electronic. The medical document is valid for 12 months. If its validity period has expired, you must fill out the form again.

The certificate for obtaining a voucher is of a preliminary informational nature, does not replace a sanatorium-resort card and does not give the right to enter the SKO for sanatorium-resort treatment, which can also be provided on an outpatient basis.

A certificate for obtaining a voucher is filled out by attending physicians of medical institutions providing outpatient care.

The darkened field of the certificate for obtaining a voucher (items 6-13) is filled out and marked with the letter “L” in the organizational and methodological office only for citizens entitled to receive a set of social services.

A certificate for obtaining a voucher is a document of a medical institution, the entries in which are confidential information and are not subject to disclosure, which is brought to the attention of employees of the medical institution.

On the title page of the certificate for obtaining a voucher, the full name of the medical institution is indicated in accordance with the registration document.

The certificate number for obtaining a voucher is the individual registration number of the certificate for obtaining a voucher, established by the medical institution.

Last name, first name, patronymic, gender, date of birth, address of permanent residence in the Russian Federation are filled out in accordance with the citizen’s identity document recognized on the territory of the Russian Federation.

In the paragraph “Identification number in the compulsory medical insurance system” for the compulsory medical insurance policy, the identification number is indicated in accordance with the form of the submitted policy, where twelve characters are determined for the series and policy number.

In the paragraph “Region of residence” the code of the subject of the Russian Federation in which the patient lives is indicated, in accordance with the list of subjects of the Russian Federation located on the reverse side of the certificate for obtaining a voucher.

The “Nearest region” item is filled in only if the patient lives in a territory located near the border of another subject of the Russian Federation, indicating the code of this subject of the Russian Federation.

In the paragraphs “Climate in the place of residence” and “Climatic factors in the place of residence”, digital codes are indicated in accordance with the list of climates in the place of residence given on the back of the certificate for obtaining a voucher.

The “Benefit Code” item is filled out in accordance with Chapter 2 of the Federal Law of July 17, 1999 N 178-FZ “On State Social Assistance”. A list of categories of citizens entitled to receive a set of social services, indicating the codes, is given on the back of the certificate for obtaining a voucher. The specified item is filled in by placing zeros before the first significant digit.

Example: if a citizen who has the right to receive a set of social services belongs to the second category, “002” is entered in the “Benefit Code” item.

In the paragraph “Document certifying the right to receive a set of social services,” an entry is made according to the details of the submitted document (number, series, date).

In the paragraph “Insurance number of an individual personal account (SNILS)” the insurance number of an individual personal account is indicated on the document certifying the right to receive a set of social services.

In the paragraph “No. of the medical history or outpatient card”, the registration number of these documents, established by the medical institution, is indicated.

The “Diagnosis” item is filled out in accordance with ICD-10 (International Classification of Diseases) according to medical documentation about the forms, stages, and nature of the disease.

In the paragraph “Disease for the treatment of which the patient is sent to a sanatorium,” the diagnosis of the disease for the treatment of which the patient is sent to a sanatorium is indicated.

The paragraph “Main disease or disease causing disability” indicates the diagnosis of the main disease, and for disabled people and disabled children, the diagnosis of the disease causing disability.

The item “Concomitant diseases” indicates diagnoses of concomitant diseases.

In the “Recommended treatment” section, the attending physician makes a note in accordance with medical indications.

The items “Preferred place of treatment” and “Recommended seasons of treatment” are optional.

The certificate is certified by the signatures of the attending physician, the head of the department or the chairman of the Institution and the round seal of the medical institution.

Why is it necessary?

Certificate 070/4 is needed for those people who suffer from illnesses and need a sanatorium-resort holiday. This medical certificate is issued by a medical institution for subsequent provision at the place of employment, study or to social security authorities.

A medical document can serve as the basis for issuing a patient a free or discounted voucher to a boarding house. However, the medical certificate is not mandatory, but advisory. That is, the document does not provide a 100% guarantee that a person will be provided with a free ticket to a sanatorium.

There are categories of citizens who need treatment more than others and can receive a discounted voucher to a boarding house:

  1. war participants;
  2. labor veterans;
  3. disabled people of different groups.

The certificate must be completed only by specialists with medical education! Neither the employer, nor the social security authorities, nor the patient himself has the right to fill out the form independently.

To obtain a certificate form 070/y you will have to undergo a medical examination. Be sure to see a therapist. Additionally, the therapist can give directions for tests and examinations. You will also need to go through those specialists who examine the diseases that the patient suffers from. There is no mandatory list of specialists that a patient must undergo in order to receive a document of the established form.

When is certificate 070/у needed?

Document 070/у is issued to obtain a voucher to a sanatorium for health procedures. The primary task is to refer the patient not to any medical institution, but according to the profile of existing diseases. To do this, the paper lists information about the main and concomitant diseases. In accordance with them, the doctor indicates contraindications, gives recommendations on nutrition, physical activity, climate, etc. This information is required to choose the right course of treatment in a sanatorium, which will have a beneficial effect on a person’s health and, most importantly, will not harm.

Form 070/у is required for certain categories of citizens who are issued preferential vouchers:

  • disabled people;
  • children with disabilities;
  • WWII veterans;
  • combat veterans;
  • people awarded the badge “Resident of besieged Leningrad”;
  • people who worked in the rear during the Second World War - in the construction of defensive structures, front-line sections of railways and roads, naval bases, airfields, air defense facilities, etc.

Such a medical certificate will be needed for those who are going to receive a discounted voucher. For example, its cost is compensated by the Social Insurance Fund, trade union organization or employers. Sometimes vouchers are issued under special municipal or state programs, etc. If a person purchases tickets to a sanatorium at his own expense, this document is optional.

IMPORTANT!

Paper 070/y is valid for 1 year after it was issued to the patient at the clinic (this period is indicated on the form itself). A person gives it to the social security authorities, trade unions, employer, etc. when submitting an application with a request to provide a ticket to a sanatorium.

What does the document look like?

Certificate form 070/у is performed on a special form. The document can be presented on paper or issued electronically. According to Order of the Ministry of Health No. 834n, the form is filled out by a health worker, certified by the attending physician, and approved by the head of the department.

The form must inevitably have a round official seal. Also, each medical certificate is assigned its own personal number, which helps to avoid forgery and falsification of the document by unscrupulous citizens.

The information form must contain the following information::

  1. complete data and residential address of the patient;
  2. name of the medical institution;
  3. for persons with disabilities, a special mark is placed on the form;
  4. SNILS data, as well as health insurance policy number;
  5. patient's medical card number;
  6. diagnosis of the disease, which is given in the form of coding;
  7. information about the presence or absence of chronic diseases, head and back injuries;
  8. list of diseases;
  9. possible contraindications to treatment;
  10. recommended treatment and its conditions;
  11. preferred location for proper treatment.

The certificate confirms the diagnosis and the presence of diseases, but does not guarantee that the patient will be issued a free or discounted voucher.

Important! The electronic document is signed with the electronic signature of a medical employee.

Document structure

The rules for drawing up and filling out a certificate are regulated at the legislative level. They are spelled out in the appendix. 12 of the order of the Ministry of Health No. 834n dated December 12. 2014, latest edition dated January 9. 2022

The document cannot be drawn up in free form. A universal form is provided to fill it out. It is attached in appendix. 11 to the order of the Ministry of Health No. 834n. The help consists of three sections. Their description is presented in the table:

What is filled in the title section

The document header contains the following details:

  • the full name of the healthcare institution where the certificate was issued;
  • the exact address of the location of the institution (indicated in the form: short name of the subject of the Russian Federation, locality, street, house);
  • code of form and organization according to the All-Russian Classifier of Enterprises (OKPO);
  • serial registration number of the document assigned during preparation (required for maintaining internal records of the institution);
  • Date of preparation.

What information is entered into the main columns of the document?

The second section contains the citizen’s personal data and the main recommendations of the treating doctor:

  1. FULL NAME.
  2. Floor.
  3. Date of Birth.
  4. Registration address (if it differs from the actual place of residence, then information about two addresses is entered).
  5. Details of the compulsory insurance policy.
  6. Information about the region in which the recipient of the certificate lives. The columns contain code values ​​that define:
      serial number of the subject of the Russian Federation, its climate;
  7. serial number of the nearest region and its climate;
  8. Code value of the state support measure, in connection with the registration of which a certificate is issued (if the recipient is a beneficiary).
  9. Item about accompanying a citizen (the field is filled in if the recipient of the certificate is a citizen with a 3rd degree disability or a child with an assigned disability group.
  10. Details of the certificate or identification on the basis of which the citizen was provided with a set of social services.
  11. Registration number of the pension certificate (SNILS card).
  12. The number from the medical institution’s accounting system under which the citizen’s outpatient card is entered.
  13. The diagnosis and its code according to the international classification.
  14. A note indicating whether the illness or injury is related to a previous injury to the brain or spinal cord.
  15. If a citizen is assigned a disability group, then the name of the diagnosis in connection with which it was assigned is indicated.
  16. Name of accompanying diagnoses (if any).
  17. A note about the recommended method of treatment (in a sanatorium or on an outpatient basis).
  18. A mark about the time of year when treatment procedures are recommended (if the season does not matter, then all four points are marked).
  19. Full name, position and handwritten signature of the doctor.
  20. Full name and signature of the head of the department of the medical institution.

Reverse side of the form

On the reverse side of form 070/U there are three tables. They contain code values ​​used when filling out basic data. The tables show the numbers:

  • climates and climatic factors;
  • regions of the Russian Federation;
  • categories of benefit citizens who have the right to receive a free voucher.

Filling rules

When drawing up certificate 070/U, you should take into account the specifics and follow the rules. Their list is as follows:

  1. The certificate is drawn up on a unified form exclusively on paper. It is not yet possible to issue it in an electronic copy.
  2. There should be no errors in the personal data of the recipient of the document, in the diagnoses and in the doctor’s recommendations.
  3. Corrections on the form are prohibited.
  4. In the columns where the main and concomitant diseases are indicated, instead of their names, it is allowed to use code values ​​according to the international classification.
  5. All text must be written in legible block letters.
  6. The form must contain the seals of: the therapist who issued the document;
  7. specialist doctors who made recommendations for sanatorium treatment;
  8. the head of the department of the medical institution where the certificate was issued.

Where can I get it?

You can receive a certificate form 070/у for free. To obtain a medical certificate, you need to contact the clinic at the patient’s place of registration or residence.

The following documents may be required for registration::

  1. passport with registration stamp;
  2. if there is no permanent registration, you can provide a temporary registration form;
  3. medical insurance;
  4. SNILS;
  5. medical card.

Often foreign citizens or stateless persons, refugees apply for a medical document. Such persons also need to present the necessary package of papers to receive a medical form, form 070/у.

Thus, foreigners and stateless persons must present an identification document, a temporary residence permit, a residence permit, as well as other documentation provided by a particular medical organization.

Not only adult patients, but also children can apply to the institution to purchase a certificate. Often, minors may need medical documentation to obtain a trip to a sanatorium at their place of study.

Where to get it

There are many options for where to obtain a medical certificate 070/u: the patient has the right to go to both a state outpatient medical institution - a district clinic, and a private medical organization. In the second case, the preparation of this document is a paid service.

To fill out the paper in form 070/у, you must go to the therapist of the selected medical institution with all available medical documents. If the therapist is the citizen’s attending physician or if the provided medical documents provide comprehensive information about the patient, one doctor is sufficient to complete the document. What to do if the patient is not registered at the clinic

In practice, there are situations when a person needs sanatorium-resort treatment, but is not assigned to a clinic and is denied a document. Indeed, you can only obtain an established and completed Form 070/y from your attending physician. However, you can be assigned to a clinic not only by registration, but also by place of residence or stay. To obtain a medical certificate, you must register at the clinic and then make an appointment with a therapist.

You can avoid queues and long waits for document production if you contact a private clinic that has the appropriate license . The law does not prohibit receiving a certificate of form 070/y from a private medical institution. However, you will have to pay a certain amount for producing and filling out the form. But the form will be prepared in the shortest possible time and you won’t have to sit in long lines.

When choosing a private clinic that provides paid medical services, you need to start not from the prices of services, but from the presence of a license. If a medical certificate is issued by an institution that does not have the proper license, the document will be considered invalid. The patient will not be able to undergo sanatorium-resort treatment on a document declared invalid.
