Health passport: what is it?

A person who gets a job with hazardous working conditions receives a health passport. The list of professions where an employee needs a document is regulated by Order No. 302N of 2011. This is a book where the results of examinations and the conclusion of the medical commission are entered year after year, starting from employment and then annually.

Unlike a medical sanitary book, a health passport is issued by a health care institution at the place of residence. During the commission, the document is in the clinic, then it is handed over to the employee. The passport has an individual number, which is entered in a special journal. In case of loss, the patient is issued a duplicate passport.

Important! If a person needs both documents at once, then one examination is carried out, and the results are entered into both documents.

Legislative framework for medical examinations

The obligation to undergo medical examinations is enshrined in Article 212 of the Labor Code of the Russian Federation. The next article of the code determines that such a requirement applies to categories of workers who:

• work in harmful or dangerous conditions;
• are engaged in ensuring regular transport traffic;
• work in preschool institutions, schools, clinics, hospitals;
• are employed in catering and retail trade, and work in food manufacturing enterprises.

Article 76 of the Labor Code determines that an employee who has not passed the inspection does not have the right to begin performing job duties. The procedure for undergoing a medical examination, as well as the list of harmful and dangerous types of work, is regulated by Order of the Ministry of Health of the Russian Federation dated April 12, 2011 No. 302n.

A medical organization of any form of ownership can carry out a medical examination. The contract must contain the following mandatory information:

• information about the availability of a license permitting medical activities;
• list and cost of paid services;
• conditions and terms of implementation;
• settlement procedure between the parties.

Preliminary, periodic or extraordinary medical examinations of employees are carried out at the expense of employers.

Every employee should know what a medical compliance passport looks like. The document has a personal number, the sequence is maintained in each medical institution. The document must necessarily contain several categories of data, otherwise it may be declared invalid:

• personal information of the employee: gender, date of birth, phone number;

• name of the clinic issuing the document;
• address of the medical institution at the place of residence where the person is served;
• information about the employer (name, form of ownership);
• applicant's position;
• class of hazardous working conditions with an extract of points;
• work experience in the industry;
• information about previous places of work.

If an employee changes his last name, it is entered next to the old data. All fields are filled out by the registrar.

As the commission passes, specialists place stamps on the free pages of the passport. In the appropriate column, the chairman of the commission provides an opinion on the suitability of the employee.

A referral to undergo a medical examination is issued to the employee against signature, in accordance with the requirements of paragraphs 7 and 8 of the Procedure. The referral form does not change depending on the type of medical examination. It should be noted that there is no unified form.

The following information must be included in the referral:

• information about the organization that sent the employee for a medical examination (name, legal form, type of activity code);
• information about the medical organization that conducts the medical examination;
• type of inspection;
• Full name, year of birth, department and position of the employee;
• list of possible dangerous and harmful factors;
• details of the person who issued the referral, his position and signature.

The list of employees who are sent for medical examination is compiled in 2 copies. The first remains in the medical institution, the second is sent to the territorial body of Rospotrebnadzor.

How to get a health passport

A person learns what a health passport is when he goes to a clinic at his place of residence or stay. The required list of documents is small: a referral from the employer and a civil passport. Some types of activities also require the provision of a certificate from a narcologist-psychiatrist.

In parallel with the registration of the book, a medical record of the outpatient is created. The start date of the examination is determined in accordance with the plan by the management of the clinic.

The list of persons subject to passport registration includes the following citizens:

• students, persons under 21 years of age;
• working in basements and underground;
• involved in trade;
• kindergarten teachers;
• medical workers;
• employees of a water tower, water utility or water production plant;
• food industry workers;
• those involved in the transport sector (professional drivers);
• chemical industry workers;
• representatives of the metallurgical industry;
• oil production sector;
• power plant employees;
• representatives of the pharmaceutical industry.

Students entering specialties from the list must also be certified. This is due to the fact that the practice takes place in the conditions of kindergartens, hospitals, clinics, and catering centers. After the initial submission of documents, the clinic becomes responsible for processing and entering data.

The organization compiles a list of people subject to medical examination. Then looks for a clinic certified to perform this task.

Important! The medical examination is paid, but usually the organization pays the cost of the examination for the employee.

The medical institution draws up a schedule for passing the medical commission and submits it to the employer for approval. workers are introduced to the examination plan, given referrals for commissions and tests. If a pathology is detected, the employee undertakes to inform the boss about it.

What additional information will be used during inspections?

The possibility has been introduced to take into account, when conducting a preliminary or periodic medical examination, documented results of previously conducted (no later than 1 year):
- preliminary or periodic examination,

- clinical examination,

— other medical examinations.

In addition, medical centers can, in order to clarify the diagnosis of those starting work, request information of interest about the health of the person being examined from the clinic at his place of residence or attachment.

Who is responsible for issuing the passport?

Filling out the title page indicating the date is carried out at the registry of the state clinic. It must have a license to provide this type of service to the public.

If an erroneous entry was made during the examination, corrections and deletions are not allowed. In the appropriate column, the medical professional must make a note “the entry is erroneous” indicating the date and time the changes were made. It must be certified by 2 signatures of responsible persons and the seal of the institution.

Order 302n strictly regulates only the list of data that is indicated on the title page. There are no strict rules for filling out the remaining sheets, so the form is free.

Rules for issuing a health passport

How long will it take to complete the document?

The speed of passing the commission depends on the list of tests and specialists who need to be visited. A sample list includes:

• measurement of height, weight;
• general analysis of urine and blood;
• blood pressure measurement;
• biochemical study of biological fluid;
• blood test for HIV, parenteral hepatitis;
• after 40 years, cholesterol test;
• starting from 45 - study for tumor markers;
• fluorography;
• mammologist - for women, urologist - for men;
• therapist;
• surgeon;
• neurologist;
• ophthalmologist;
• fluorography.

The commission can last 2-3 hours, since tests can be taken on an emergency basis. Subspecialists can quickly assess the presence of a serious pathology. If doubt arises, additional research methods will be needed: ultrasound, x-ray, MRI. The maximum commission period is 1-2 days.

After the conclusion about fitness for work, the employee is given a medical passport, after which he must submit it to the human resources department. An employee responsible for monitoring the timing of the medical examination is appointed on site. The passports are kept by him.

At the discretion of the head of the organization, medical documents may be kept by the employee himself.

The appearance of employee health passports is a serious step towards improving the health of the country's residents.
