Form 6 is an application for registration at the place of residence from an individual who has arrived in a new place.
Its sections contain the following information:
- name of the registration authority;
- Full name and passport details of the applicant;
- previous residence address;
- when registering minors under 14 years of age, identification data of their legal representative: parent or guardian;
- address where you need to register;
- basis for registration: reference to the rental agreement for living space or to the application of its owner;
- Full name and passport details of the owner of the property;
- confirmation of the authenticity of the signatures of the applicant and the owner of the living space.
The document is divided by a dotted line into two parts: the upper one is filled out by the applicant, the lower one – together with the employee of the body carrying out registration.
The lower part is intended for forwarding to the passport registration authorities at the previous address and contains the following information:
- name of the accounting unit at the old address;
- Full name and passport details of the registered person;
- identification information of parents or guardians;
- new registration address;
- request for deregistration.
The lower part is sent by the registering organization; the citizen does not need to take care of it.

Registration is carried out both for citizens living on social rent and for housing tenants. Owners of residential real estate for rent or officials responsible for their operation are also responsible for the timely registration of residents.
Registration of minors has its own characteristics:
- children under 14 years of age are registered by their parents or guardians, and they fill out an application for child registration;
- children from 14 to 18 years old who have received a passport fill out the application independently, but they can only be registered with one of their parents or guardians;
- when changing the place of residence of children, approval is required from the guardianship authorities; they ensure that the minor’s rights are not violated and his living conditions do not worsen;
- newborns are registered within seven days from the date of receipt of the birth certificate at the address of one of the parents.
As a rule, this is the mother's address. If the parents are registered in different places, the father will need the mother's consent to register. The separate consent of the owner of the property for the registration of a minor with his parents is not required.
Algorithm for submitting an application for registration
To apply for registration, the first thing you need to do is arrange all the documents, find and fill out the application form.
You can submit your completed application directly to the relevant department of the migration service. Currently, its functions have been transferred to the Main Directorate for Migration Affairs under the Ministry of Internal Affairs.
The document can be submitted in person, through a representative, or sent by mail.
Besides. it can be submitted through the public services portal, MFC. In the latter cases, you will still need to obtain a registration certificate for children and a passport stamp for adults at the FMS department.

How to write an application for registration
There are three ways to fill out Form 6:
- Manually. In this case, the applicant enters the data into the columns of the form taken from the MFC or the migration service department and signs it.
- On the computer. You need to download the application template online and enter information in its columns. The completed form is printed and certified by the handwritten signatures of the applicant and the owner of the residential premises.
- Online, on the State Services portal. To fill out an application on the State Services portal, you will need to create a personal account on it and issue an electronic signature. Their registration takes some time, it is better to take care of this before moving.
Note! All three methods of writing an application have equal validity, but they have different deadlines for consideration and decision-making by the registration authorities.
Form No. 6 of the application for registration - what is it?
A special form has been approved for registration of citizens in residential premises. It is an annex to the Regulations of the Migration Service for the implementation of registration. Please note that the form is also used in case of deportation from the previous place of residence.
This document is called Form 6, and consists of two parts. The first is filled in when it is necessary to register, and the second in case of deregistration. This form is used specifically for accounting when citizens change their place of residence.
Rules for filling out the form
When drawing up F-6 manually, you should use a ballpoint pen. Felt pens and pencils are not suitable, such an application will not be accepted.
You must write legibly, letters and numbers must be read unambiguously, without possible double interpretation. People with illegible handwriting are advised to write in block letters. Any blots, erasures or corrections are also unacceptable. In this case, the application will have to be rewritten.

When filling out the form on a computer, the problem of legibility is eliminated; all the data on the screen can be easily checked with documents and corrected in a timely manner in a text editor program. Who and when must report to the military registration and enlistment office on Form 6
Russian employers-legal entities, regardless of their form of ownership, must keep records of those liable for military service and submit reports to the military commissariat (clause 1, article 8 of the Law of May 31, 1996 No. 61-FZ “On Defense”).
Standard annual reports for those legal entities that maintain military records are Forms 18 and 6. Form 18 is devoted to basic information about the organization and the results of its work in maintaining military records. Form 6 provides information about reservists and reserved citizens.
It is possible not to submit Form 6 if there is no data to fill it out (the company has neither reservations nor reservists). There is no need to submit additional explanations to the military registration and enlistment office, since it is mandatory to submit Form 18: it shows exactly why Form 6 is not submitted.
Military registration and enlistment offices do not require submission of reports electronically. All forms can be prepared and submitted on paper.
The report on Form 6 is prepared on the first day of the year following the reporting year and is submitted no later than the last day of the reporting year. Thus, Form 6 as of 01/01/2020 should be submitted to the military registration and enlistment office no later than December 31, 2022.
Processing times, cost
If there are no errors or inaccuracies in the registration papers, the local FMS office will complete the registration within three days. In the “Decision made” column, a record of registration at the current place of residence is made, and the grounds for such a decision are indicated. A registration stamp is affixed to the appropriate page of the passport indicating the address and code of the department that registered the resident. Children under 14 years of age who do not have a passport receive a registration certificate.

If the application was submitted through the passport office, management company, State Services portal or MFC, then the registration period allowed by law increases to 6 days. If there is a shortage of any papers from the required package or if inaccuracies are detected in the data, the countdown of the deadline is suspended until the comment is eliminated.
There are no fees or charges for registration actions; the service is provided free of charge for the applicant. Attempting to charge fees for such activities is a clear sign of fraud or malpractice.

The cost of registration at the place of residence. Deadlines. Refusal of service, features of its provision
Registration of a person, including the submission of a request for registration at the address of residence, is a free procedure, that is, the transfer of state fees is not provided here.
The deadlines for obtaining registration are regulated by clauses 32-34 of the Regulations and amount to the following duration:
- When submitting a request to the registration office with your own hand, migration service employees are required to register the person within 6 days.
- When sending an application through the State Services website, registration is carried out one day from the date of delivery to the registration office of the document, which is considered the basis for registration.
- If a document that relates to the basis for settlement is not provided, then the total registration time will be carried out within 8 working days from the date of receipt of the request.
Clause 60 of the Regulations states that the legislation does not provide for reasons for refusing to register an individual at their address of residence. The reason for the delay in registration, when sending an application through the State Services website, may be the failure of an individual to appear within 3 days after his call with original documents.
At the same time, it should be noted that refusal to register a person is possible for the following reasons:
- If a passport or other identification document is invalid (there is no photo that must be attached at the required time, there are corrections or the document is damaged).
- If the information displayed on the form is not correct.
In fact, registering at a new residential address is not difficult. Migration structures organize their activities so that any person can quickly get an appointment without any queues. Clause 38 of the Regulations stipulates that a single window visitor should not be detained for more than 15 minutes.
Deadlines
Temporary stay in another city does not oblige people to register within 90 days. After this period has expired, you must register and obtain a temporary registration, which is issued within a week for 3 months.
If you plan to stay longer than this period, a permanent type of registration is required.
When a person living and registered in Russia wishes to change his place of residence, he has 7 days from the date of arrival at the new place to obtain registration on permanent terms. After the completed procedure, a corresponding stamp is placed in the passport with information about the new address.
Registration without an extract from the old place can take from 2 weeks to one month. If you have no time to wait, waste precious time and spoil your nerves, you can deregister yourself in advance.
General filling rules
In order to avoid mistakes when entering information into the KM-6 form, you must follow the general rules:
The first line of the report should contain the name, address (legal or actual - it doesn’t matter, the main thing is that it is the same in all reports) and telephone number of the enterprise. If there is a separate division, then its name must also be indicated.
If the KM-6 form is filled out by an individual entrepreneur, then he must indicate all the necessary information in the same order. This is a common mistake - many individual entrepreneurs believe that they can deviate from the unified form, because they work for themselves. The document must indicate the name, registration number and number of the CCP manufacturer. You must indicate the exact date of completion and certificate number. Revenue must be reported in numerical form and in words. The signature of the author of the certificate report is a required element.
Important! If the proceeds are not handed over to the company’s chief cashier, but to the bank, then this must be noted in the report.
Features of the help report
The cashier-operator's certificate-report has several characteristic features:
- the KM-6 form, approved by the State Statistics Committee of the Russian Federation in 1998, is unified;
- errors when filling out, as well as deviations from the generally accepted format, may result in a fine during the work of inspection bodies;
- KM-6 must be filled out daily (or at the end of each shift), and simultaneous filling out of certificates for 2 or more days/shifts is not allowed;
- The document must be submitted simultaneously with the proceeds - either to the chief cashier of the company or to the bank;
- The certificate must be kept for exactly 5 years, after which it loses its value.
Important! Column 4 is filled in only when using outdated cash register equipment. If modern devices have been installed in the organization over the past 12 years, then this section should be left blank.
Why is the KM-6 form needed?
Certificate form KM-6 is the main reporting document of the cashier. It indicates the revenue received and is handed over to the manager along with the revenue. If the proceeds are given to collectors, then a corresponding note is required in the certificate-report.
In the work of inspection bodies, such certificates play a very important role. It is on their basis that a conclusion is made about the completeness of revenue reflection. In addition, information from the KM-6 certificate is used to fill out a summary report. This report includes data on the status of cash register meters and the income of the enterprise.
