Regardless of the form of ownership of the enterprise and its affiliation, the legislation provides for certain documentation, the maintenance of which is mandatory. These include military registration, the maintenance of which is the responsibility of all companies operating in our country.
According to the Procedure for maintaining military records, an integral part of it is the preparation of forms of various types, each of which has its own approved procedure for filling out and method of submission to regulatory authorities. In 2022, implementation standards, as well as reporting forms, remained unchanged compared to the previous year.
Form 6
The obligation to compile registration data of those liable for military service arose for each organization on the basis of Federal Law No. 53. The legislative act obliges each company to begin activities to maintain a registered license from the moment of its registration.
And also, the rules for maintaining the Unified State Register of Legal Entities, which operate on the basis of Resolution No. 438 of the Government of the Russian Federation, establish that the body registering the company is obliged to transfer data about the newly created legal entity to certain authorities within the prescribed time limits. The list of these structural bodies also includes the commissariat of territorial subordination.
In turn, the military registration and enlistment office, having received information about a newly registered enterprise, automatically registers it. From this moment on, the company administration has an obligation to maintain relevant documentation and provide timely reporting.
According to the law, companies that have assumed the responsibilities of employers have the following obligations to the military registration and enlistment office:
- Account for your employees according to office work instructions.
- Provide timely information about all changes occurring in personnel matters.
- Transmit data on the current state of personal accounting matters.
Personnel officers of companies have different views on the complexity of reporting to the commissariat. For some, it does not raise questions, but there are times when an employee is faced with a difficulty in how to fill out the form, which can only be resolved with the help of the law.
Each HR inspector, when performing his duties, must be guided by the following standards:
- Federal Law No. 61 “On Defense”, namely, Article 8.
- Federal Law No. 31 – Article 9 “On mobilization”.
- Regulations on military registration at the enterprise.
In accordance with these standards, the chief and the personnel officer are responsible for carrying out work on interaction with the commissariat. According to the regulations, it is the personnel officer who is appointed responsible for such work, but in large enterprises a separate responsible person can be identified who will deal only with HR issues.
Each organization maintains a military records file.
The procedure for determining responsible persons is as follows:
- For a number of employees of up to 500 people (members of military service) – 1 employee (usually a part-time personnel officer).
- If there are from 501 to 2000 persons liable for military service, one specialist is appointed.
- Over 2000 persons – two authorized persons.
The assignment of duties occurs by order. The direct work of the appointed person is to compile F6 for the district military registration and enlistment office. Form 6 for the military registration and enlistment office is called “Report on the number of working and reserved citizens in reserve.”

maintaining a file index of organizations and filling out organization registration cards (form No. 18)
1. A file of organizations is maintained by the regional commission for the reservation of citizens in reserve, and consists of organization registration cards (form No. 18), placed in alphabetical order by name of organization. The file cabinet should be divided into two sections:
section A – organizations where only military records of citizens in the reserve are kept;
section B – organizations where military registration and reservation of citizens in reserve are maintained.
2. The organization’s registration card is filled out and submitted to the district commission by all organizations, regardless of organizational, legal forms and forms of ownership, located on the territory of the municipality.
3. Organizations’ registration cards are registered with the district commission in the register of organization registration cards (indicating: serial numbers, name of the organization, section of the card index to which the organization is assigned, and the date of initial registration). The registration number of the organization corresponds to the serial number of the entry in the registration journal.
After assigning an organization to section A or B of the card index, the registration number additionally indicates the section of the card index (for example, 121-A).
The organization's registration card is filled out in two copies. The copy that remains in the organization’s file is marked with a registration mark by the district commission.
4. Paragraph 8 indicates the name of the government body, local government body or organization (for example, Gazprom OJSC) depending on the departmental subordination of the registered organization. If there is no departmental subordination, paragraph 8 is not completed.
5. Paragraph 9 indicates the main codes of the organization*.
6. According to clause 10.1-10.4 for non-reservable organizations (card files of section A), line 23 of form No. 6 is filled out in the district commission. For organizations that only maintain military records of citizens in the reserves, subclause 10.5 is not completed.
7. Item 12 of the organization’s registration card is filled out only for organizations that carry out reservations for citizens staying in reserve. At the same time, for organizations that are in the sphere of jurisdiction or whose activities are connected with the activities of a federal government body, organization (for example, OJSC Gazprom), the number of the list of positions and professions is indicated, according to which citizens who are in the reserve working in these organizations are reserved, and for all other organizations, the date and number of the resolution of the territorial commission on the organization of reservation and sections of the Territorial List of Positions and Professions for which citizens in the reserve are reserved are indicated.
For organizations that only keep military records of citizens in the reserves, paragraph 12 is not completed.
8. In the case of deregistration of an organization with a district commission (liquidation of an organization), the line “Note on deregistration (liquidation of an organization)” indicates the reasons for deregistration (date and number of the decision of a higher organization, decision of the liquidation commission or other reasons).
The line about the liquidation of the organization is filled out in the district commission.
9. In the process of maintaining a card index of organizations, the information on the organization’s record cards is annually updated within the deadlines established for the submission of the report.

The procedure for filling out F6 for military registration
The employee appointed by the order is obliged to comply with the deadlines for submitting reports and the rules for filling out the required forms. A responsible citizen must remember that the deadline for filing F6 is annually until December 31. The data provided in this form must be current as of January 1 of the year following the reporting year.
There is administrative liability for failure to provide reports. The degree of punishment depends on the offense committed. Violations include:
Civil liability of military personnel
- Providing false information.
- Violation of reporting deadlines.
- Making a mistake.
In addition to these violations, employers are responsible for failure to notify subordinates about their summons to the military registration and enlistment office or failure by the employee to provide documents necessary for military registration. The penalties are regulated by the Code of Administrative Offences, according to which the imposed fine varies from 300 to 1000 rubles.

Organization registration card (form 18): what is it?
“Organization registration card” (form 18 – you can download the form below) is a document that is submitted to the military registration and enlistment office and contains information about the company’s employees liable for military service. In general, this document is filled out once every two years and submitted before the end of the current year (until December 31).
The form of the organization's card is given in the letter of the Ministry of Culture of Russia No. 344-01-39-VA dated October 21, 2015. The appendix to the letter also defines the detailed procedure for filling out the card - which organizations should fill out the document, what information to indicate in this or that paragraph. The 2022 organization registration card is filled out according to the form approved in 2015 - the new form for submitting information was not approved, so the same form should be used to submit information to the military registration and enlistment office.
The organization's registration card (sample with the entered information) will be provided at the end of the article. The form is quite voluminous - you need to fill out an A4 sheet on both sides.

Military registration form 18
Form 18 is also an integral part of military registration at an enterprise. The form was introduced in 2015 and is still in effect today. There was no information about modifications this year.
- F18 displays data about persons working at the enterprise and included in the reserve. Companies, according to the profile of accounting in the organization, are divided into two types:
- companies with a person appointed responsible for conducting military registration;
- an authorized person who has the competence to reserve jobs for citizens in reserve.
Universities, when compiling Form 18, are required to indicate, at the same time as employees, the data of undergraduate and graduate students.
The frequency of submitting the form to the district military registration and enlistment office is once every two years. According to the rules, it must be submitted by December 31 of the current year. The information recorded in the form must contain data that is current as of January 1 of the following year.
Features of filling out form 18
Form 18 displays the company’s employees, citizens of the Russian Federation, who are in reserve. Organizations based on the method of keeping records are divided into two types: those who simply keep records and those who make reservations for citizens who are in reserve. In addition to employees, higher education institutions also list final year students and graduate students. Form 18 is completed every two years and submitted to the relevant government authorities by December 31 of the current year as of January 1 of the next year.
The card is filled out carefully. The document must contain reliable information. After filling out, the director signs three copies of Form 18, the signature is sealed. One copy is sent to the military registration and enlistment office for verification. After the verification procedure, the district military commission registers this document and puts a registration mark on the organization’s copy. A third copy of the form is sent to the City Council Reservations Committee.
Filling out form 18 for military registration
The algorithm for entering information into cards can be found in Appendix No. 2 of the Ministry of Culture letter No. 344-01-39-VA.
F18 must be filled out on double-sided A4 sheet; Excel documents are accepted. Among the nuances that should not be neglected when preparing reports, the following examples can be noted:
- Clause 4 specifies the last date of registration (re-registration) of the certificate of entry into the Unified State Register of Legal Entities. The full name of the tax authority that issued the document, the series and number of this certificate are required.
- Clause 10 does not take into account part-time employees. In this case, in subclause 10.1 the sum of the values according to paragraphs is indicated. "a" and pp. "b". The number of employees reflected in paragraphs. “b” of this paragraph, having a category of fitness for military service for health reasons “B”, are shown separately in paragraphs. "V".
- Clause 10.4 indicates the total number of males aged from 18 to 27 years (only citizens of the Russian Federation) who are required to be registered with the military and are not in the reserves. In paragraph 10.1, these citizens should not be taken into account.
- Clauses 10.5, 12, 15 are intended to be filled out by organizations authorized to make reservations for citizens.
Step-by-step instructions for filling out the card
When filling out the form, pay attention to the order in which information is placed according to the sample.
In clause 1 we indicate the full name of the enterprise.
In paragraphs 2-7 we write down information about the manager (full name, position, telephone number), information about the person in charge (full name, telephone number), address of the enterprise (legal, actual, postal).
We fill out item 8 if there is a higher organization. If there is none, then we leave this item blank.
In clause 9 we indicate the main codes of the enterprise in order, as they are marked in the form, as well as the organizational and legal form, form of ownership (digital code and decoding).
In paragraph 10 we write the total number of employees; part-time workers are not included in the report.
In clause 10.1 we indicate the total number of citizens: males in the reserves, and females with military specialties. We divide this figure into subparagraphs “a” and “b”, according to military ranks; from subparagraph “b” we calculate the number of citizens who have category “B” of fitness for military service (limited “fit”).
Clause 10.2 is filled out by enterprises that book citizens. The number of reserved citizens is entered here.
In clause 10.3 we enter information on the number of employees who have mobilization orders. In this case, the citizens specified in clause 10.3 are also taken into account in clause 10.5.
Clause 10.4 includes information about the presence of persons of conscription age (men aged 18-27 years old who are required to be registered with the military and are not in the reserves). Employees specified in clause 10.1 are not taken into account.
In clause 10.5 we enter information about the number of unbooked citizens. This paragraph will coincide with the information in clause 10.1.
pp. 12, 15.1 and 15.2 are filled out by organizations that book reserve employees. It should be noted that clause 12 contains information about the types of economic activities and positions for which the reservation is being made (according to the List of Positions and Professions). If the activity of the enterprise is related to or is under the jurisdiction of a federal government body, only the List number is indicated.
Upon liquidation (change of legal address), the company is deregistered. An entry about this is made on the card in the district commission in the line “Note on deregistration (liquidation of the organization).”
Organization registration card for the military registration and enlistment office
The legislation provides for two types of military registration for legal entities: simple and with the possibility of booking jobs, that is, F18 must necessarily contain this information.
When registering an enterprise, the F18 registration card is initially filled out and submitted to the district military registration and enlistment office and the specialized reservation committee. The deadline for submitting primary information is two weeks from the date of state registration and entry of data into the Unified State Register of Legal Entities.
Then, at intervals of two years, F18 is filled out by making changes to the primary information (if any). New information must not contradict information already transmitted. Upon initial submission of F18 to the reservation commission, a registration entry is made in a special journal with the assignment of an individual number. This assigned code is stamped on all forms submitted subsequently.
Who and when to give it to?
Companies must fill out the military registration record for the first time and submit it to the territorial military registration and enlistment office and the district reservation commission within two weeks from the date of state registration and entry of information into the Unified State Register of Legal Entities. Further, every two years the card is expected at the reservation commission and the military commissariat until December 31 of the reporting year. Information in the report must be provided as of January 1 of the next year. In addition to information about employees, educational organizations are required to submit information about students and graduate students.
The document is always drawn up in triplicate:
- For the military registration and enlistment office.
- For commission.
- Remains in the organization.
When the card is initially sent to the reservation commission, it is registered there in a special journal. She is assigned a registration number, which must then be marked on all forms.
